I guess the mixture of those gives the motivation. Having payment wages but with really limited free time isn't doing much either. I picture it out as a normal curve graph in statistics. "Too right" and "too left" results lesser.

I think the first and last points you made are very important. If you can find out the motivator for individual employee and satisfy his needs he will give his best in return when you need him. Leading by example is an age old technique that every great leader has used and needs to be used if you want your employees to follow.
