How about expanding too quickly when you see initial success?

I am the manager of a small-medium size company. It started with about 50 people. Following initial success, we hired, added many new projects and moved to a big, fancy place to handle the now 120-strong workforce. Economic downturn and finding out we added things we had no experience in. 6 months later, we halved our workforce, wrote off a hundred thousand in losses and moved back into the smaller office with our tails between our legs :(
