Daniel - You have come up with a nice compilation. #5 is my favorite. Many people quit just when they are approaching the finish line. Only those who persevere are able to achieve success.

I would also add 1 more to the list. How about failing to recognize future trends and their impact on your business. The world is not still. You have to be able to see what's coming in the horizon and adjust accordingly. Failure to do so a recipe for disaster.
