One such company I know had a HR department that had a 150% turnover rate. only two members of staff had actually stayed on for over a year, and one of them was the manager. Many positions had to be refilled several times within a year. While some do not see this as a major problem - the amount of inefficiency in the office and the costs to the company must have huge.
