I use all 3 options:
If I feel they're a quality outfit that's also relevant to what I'm doing, I'll follow-back.
If I feel they're a quality outfit that's not relevant, I won't follow-back, but I won't block them, either.
If I have even the slightest indication that something ain't above board, if they're totally irrelevant, or if they have over 10k-20k tweets (depending - how many is up to my discretion), I'll actively block them.
I also block lots of @tweeters - for too many tweets, or because some are highly offensive. I block all spammers.
